This form serves as an initial enrollment form. When submitted with a registration fee of $125 (nonrefundable), your place in the program will be held for the 2021-22 school year. A more extensive enrollment packet will be issued closer to the start of the school year, with an additional supply fee of $75 (nonrefundable).
